School Forms

Welcome to St. Casimir's new School Forms page. As standard school forms are converted to electronic format they will be accessible through this page. Two formats will be provided: 1) MS Word and 2) Adobe PDF.

MS Word forms can be saved to your system for completion and printing.

Adobe PDF forms cannot be saved to your system (unless you have the appropriate software) and must be completed on line then immediately printed or you will lose the data. If you do not have Adobe Acrobat Reader you must download this free software from the Adobe website by clicking on the Adobe icon below prior to opening the file.

All forms, if required, must be accompanied by the appropriate fees. Please contact the office at 603-623-6411 for up-to-date fees prior to mailing completed forms to the office. The school mailing address is:
